Department Custom Scope
The Community Health Worker (CHW) Supervisor will be responsible for overseeing the Center?s per diem Community Health Workers and provide support to the identification, education, and coordination of community member?s health and social service needs. The Community Health Worker Supervisor will be responsible for determining appropriate outreach and scheduled activities for the Center which require the assistance of CHWs. In addition the CHW Supervisor will also be responsible for supporting community members in improving their health through outreach, educational and engagement activities either via phone and/or in the field and connecting them with appropriate social service organizations. |
