Position Purpose

This position will work in conjunction with a supervising neurosurgeon will provide clinical expertise to acutely and chronically ill neurosurgery patients by taking detailed medical histories, performing exams, interpreting lab results, making diagnoses, and implement in- and out-patient treatment. Hospital duties include assisting in neurosurgical procedures, closing wounds, ordering and reviewing MRIs and CT scans, and making rounds. Collaborates with others in the clinic to assure smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care. The care delivered will be based on theoretical principals and scientific knowledge and in accordance with requirements set forth by the California Physician Assistant Board.

The position's primary work location will be in the inland empire working in the UCR Health Clinic and in affiliate facilities. Additionally, this position may be required to be on call and work different shifts, including evenings, weekends, and holidays.

Under delegation of affiliate services agreement, scope of practice, and supervising physician, will provide clinical expertise to patients and their families. Facilitate the clinical management of neurosurgery patient by collaborating with the attending physicians and appropriate members of the healthcare team.

1. Attend rounds, participate in patient evaluation and care plan design. Order and interpret lab tests, imaging, and other diagnostic test, initiate appropriate patient care/treatment plan.

2. Facilitates admission of patients to the Neurological Surgery:
a. Elicit and document history and physical examination.
b. Write patient orders and document in the medical record.

3. Coordinate care with staff and consulting services.

4. Perform and document physical examination - health and physical (H&P) assessments. Performs assessment/data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy.

5. Monitor, evaluate, and document patient responses to interventions.

6. Reconcile medications and provide medications and devices to patients.

7. Perform preoperative evaluation; obtain appropriate studies and review prior to surgery.

8. Document interval progress notes with treatment plan.

9. Consult and evaluate patients in collaboration with the chief resident / attending physicians:
a. In the in/outpatient setting and on various units in the hospital.
b. On off-service patients as requested
c. With/refer to designated physician , medical/surgical specialty service, or other professionals.

10. Evaluate high risk and complex cases:
a. Identify cases for review with attending physician and departmental review
b. Establish individualized plan of care for rehabilitation and recovery.
c. Facilitate physical medicine and rehabilitation (PM&R) services including but not limited to occupational/physical/speech therapy, PM&R consultation.
d. Facilitate specialty services including but not limited to interventional radiology, neurology, radiation oncology, and hematology oncology.

11. Monitor, evaluate plan of care:
a. Set parameters and guidelines for continuum of care with multi-disciplinary team.
b. Evaluate and define patient discharge regimen.
c. Dictate/electronically create interval summaries on patients.
d. Coordinate/recommend follow up: Imaging and studies, neurological surgery outpatient appointments with clinic staff, consult referrals and outpatient follow up.

12. Monitor, evaluate, and implement patient transfer from neurosurgical service to other services or facilities:
a. Arrange transfer: between services, to outside facilities, (jail, hospitals, skilled nursing facilities, rehabilitation units).
b. Dictate electronically create transfer summaries.
c. Recommend follow up for consult referrals.

13. Monitor and evaluate patients in the outpatient setting with new or ongoing neurosurgical issues:
a. Respond to patient telephone calls and medical record patient encounters.
b. Set parameters and guidelines for continuum of care in collaboration with neurosurgery attending physician.
c. Dictate electronically create clinic note.
d. Coordinate/recommend follow up with appropriate imaging and diagnostic studies.
e. Recommend consult referrals.

14. Per extended role procedures including assisting in the operating room.

15. After being trained and deemed competent by attending physician - may perform ventriculostomy catheters and icp monitors.

16. Respond to emergencies using ACLS protocol as needed.

17. Respond to patient care issues/admissions when on call.

18. Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team.

19. Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information.

20. Completes charge encounters for physician approval.

A minimum of two years' experience as a PA.

Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications.
