Position Purpose

Under general supervision, incumbents provide care to patients in a structured environment; perform established nursing interventions utilizing current clinical knowledge. This is the operational level in the series where a majority of clinical nursing positions are allocated.

Under the direction of the healthcare provider, provides professional care and treatment of women's health, multi-specialty and infusion therapy patients. As a patient advocate, provide physical and psycho social support to patients, their friends and families. It is anticipated that this per diem position will work one-two days per month in 4 to 8 hour blocks.

Essential Functions

Direct Patient Care 55
1. Determines the chief complaint from the patient and records it accurately in the medical record. Recognizes symptomology of common medical illnesses and emergent medical conditions. Makes appropriate annotations in the patient chart, maintaining accurate and complete records of nursing care.

2. Applies protocol for encountering infectious clients and emergent medical conditions.

3. Prepares patients for exam with correct positioning and draping.

4. Accurately takes and records vital signs in patient medical record.

5. Assists with treatment and exams.

6. Administers prescribed procedures under provider direction including dressing changes.

7. Phone triage patients, set up and assisting with office procedures.

8. May administer intravenous, intramuscular, subcutaneous and oral medications per policy. Performs venipuncture adhering to all OSHA personal protective equipment (PPE) safety requirements. Monitors patients for change in condition and reaction to medications. Vaccinates patients who come in for routine appointments, as needed.

9. Starts and monitor IV's. Oversee the infusion therapy. Maintains supply inventory for infusion therapy.

10. Provides patient education regarding common conditions per provider direction.

11. Reviews charts for lab and X-ray results, correct completion of all lab and X-ray work, consultation requests, and accurate filing of all chart information.

12. Assists with refills of patient prescriptions by obtaining medical record, notifying physician and recontacting patient with instructions.

13. Under direction of medical provider, makes appropriate annotation in patient management system using accurate ICD-10 codes.

14. Coordinates flu clinics and any other injections clinic as directed by supervisor.
Patient Phone Triage 20
1. Primary contact for the Nurse triage phone line.

2. Manages a variety of common presenting issues using standardized procedures.

3. Evaluates and treats patients as per protocols written by providers and under the supervision of providers.

4. Documents each patient encounter in the patient's EMR, including assessment and disposition.

5. Anticipates physician needs.
Procedures 10
1. Provides patient teaching related to all procedures and after-visit care as relevant.

2. Observes condition of patient and reports unusual conditions.

3. Documents pre, intra and post procedure in the medical record.

Manages patient infusions in the specialty clinic infusion center:
1. Initiates venous access for infusion patients.

2. Prepares and administers IV medication for infusion.

3. Monitors patients throughout infusion per unique medication protocols.

4. Assists providers in ensuring patient compliance with specific infusion schedule.

5. Documents infusion and patient tolerance in the medical record.
Other duties 5
1. Establishes effective working relationships with members of the health care team, patients, and families.

2. Delegates appropriately and coordinates duties/activities of health care team members.

3. Demonstrates knowledge of and applies safety principles as identified by the clinic.

4. Performs efficiently in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events.

5. Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functions.

6. Participates in clinic performance improvement activities.

7. Identifies and solves problems effectively.
Equipment and Supplies 5
1. Operates, calibrates, and performs periodic maintenance and quality checks on electronic equipment.

2. Ensures equipment is functioning and accurate prior to patient appointments.

3. Collaborates with clinic manager when ordering/replacing equipment and supplies.

4. Cleans, sterilizes and sets up the equipment to ensure its readiness for the next procedure.

5. Ensure a clean and safe working environment.

6. Performs related technical and administrative duties to ensure the smooth and effective operation of procedures.

7. Acts as a resource person to professional staff.

8. Maintains adequate supply inventory.
Staff Education / Competency Assessments 5
1. Serves as an educator and resource to other clinical staff.

2. Develops competency checklists and assesses the competency of other staff members using the appropriate observation and documentation techniques.

Possession of the legal requirements to practice as a Registered Nurse as determined by the California Board of Registered Nursing and six months of clinical experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.

Current Basic Life Support (BLS) certification.

Familiar with and can assess the competence of others in their clinical tasks - such as point of care testing (POCT).

Excellent communication skills at level necessary for taking patients' medical histories, understanding provider and supervisor instructions, and for accurately documenting patients' medical information.
