Position Purpose
Responsible for coordinating the release of protected health information. Ensures compliance with legal requirements for authorization. Will perform audits and communicate findings with other members of the healthcare team to ensure the accuracy and completion of medical records. This position is also responsible for maintaining records in accordance with current federal, state, and local standards, guidelines and regulations that govern medical records. The coordinator assists in all areas of the health information management (HIM) functions, the primary duty being the release of information (ROI). Related duties include maintaining the accounting of disclosures database, and invoicing. Other duties include reviewing records for completion, reporting noncompliance, providing reports to providers of care, retrieving and filing of records, maintaining organization of filing areas and department, and auditing medical records for compliance as per laws, rules and regulations of federal and state licensing agencies. Inspects records to insure completeness and internal consistency in quality control and data substantiating diagnosis and treatment; maintain liaison with clinical and other users of medical records; assist in defining and maintaining standards for medical record keeping. This position will be required to travel to various clinic and affiliate locations in the inland empire. |

Coordinates the authorized release of protected health information to other healthcare entities, billing/payors, patients, attorneys, and others. Ensures all release requests comply with Health Insurance Portability Accountability Act (HIPAA) and the Confidentiality of Medical Information Act (CMIA). Keeps an accurate and detailed record of all requests and includes the authorization and communication material in the medical record. 1. Release of information per UCR policy, state and federal regulations, including HIPAA and CMIA requirements. 2. Maintains the confidentiality of medical records, communications, and proprietary information, accessing information on a need-to-know basis only. 3. Interfacing with requestors and their authorized agents via telephone and other appropriate methods. 4. Maintains appropriate logs of releases. 5. Assists with invoicing and reconciling billing and payment related to releases. 6. Performs quality assurance reviews on all released records. 7. Interacts with others (co-employees, management, physicians, patients, visitors, vendors) in a professional and tactful manner including treating them with respect and consideration regardless of their status or position. |
