Community Health Worker, Per Diem

Position Information

The Community Health Worker (CHW), per diem, will be responsible for identifying and determining social service needs for individuals in the community by conducting social determinants of health assessment within their scope through outreach and engagement activities. The Community Health Worker will be responsible for assessing the health risks and identifying gaps pertaining to Social Determinants of Health (SDoH) which create barriers and contribute to the well-being of an individual’s care needs. Under the general supervision of the Center for Healthy Communities leadership, the Community Health Worker will be responsible for supporting community members in improving their health through outreach and engagement activities with follow-ups held over the phone and/or in the field. In collaboration with the individual, the Community Health Worker will take ownership of Social Determinants of Health (SDoH) goals, and will be responsible for facilitating health, risk, and safety screenings, health education activities, and special events to promote health for community members as well as help individuals access social services for their needs.

Please Note: There are 2 per diem vacancies. Per diem, appointees may be scheduled on a pre-scheduled basis or as needed on a day-to-day basis or called off from a pre-established schedule as determined by the University. These positions are grant-funded appointments with end dates of 06/30/2024 and may be extended should funding be available.

The pay rates for these positions range from $22.00/per hour to $30.00/per hour depending on relevant experience.

Applicants must have current work authorization when accepting a UCR staff position. Currently, we are unable to sponsor or take over sponsorship of an employment Visa for staff.

As a university employee, you will be required to comply with all applicable University policies and/or collective bargaining agreements, as may be amended from time to time. Federal, state, or local government directives may impose additional requirements.

Associate degree in related area and/or equivalent experience/training. Required
Continuing education in related field. Required
Completion of approved Community Health Worker program. Preferred

Certifications

Certification Requirement
CHW State Certification when available Preferred

Experience

2 - 4 years of related experience. Required

Minimum Requirements

Working knowledge of departmental and organization policies and procedures; knowledge of educational program content and objectives.
Working skills to produce and deliver programs.
Good reading, verbal, written and interpersonal communication skills, and good presentation skills.
Working knowledge of delivering community educational programs.
Skill in using business software systems in the completion of work assignments; i.e., Word, Excel, Access, PowerPoint.
Working knowledge of current community education programs and trends.
Good analytical skills to understand how business needs can be addressed through the development and delivery of training programs.
Working knowledge of conducting needs assessment.

Preferred Qualifications

Effective communication skills to interact with various members of the community

Bilingual skills in English and Spanish.
